Welcome to Glow Med Spa. By scheduling an appointment, purchasing products or services, enrolling in a membership, or using our website or patient portal, you agree to the following Terms of Service.

1. Medical Services

Glow Med Spa provides aesthetic and wellness services under the supervision of licensed medical professionals. Individual results vary, and no guarantee of specific outcomes is made or implied.

Treatment recommendations are based on each client's individual medical history, assessment, and provider judgment. Providers reserve the right to decline or postpone treatment if it is determined to be unsafe or medically inappropriate.

2. Appointments

Appointments may be scheduled online, by phone, text message, email, or in person.

Please arrive on time for your appointment with all required paperwork completed. Arriving late may result in a shortened appointment or the need to reschedule.

3. Cancellation & No-Show Policy

Appointments must be canceled or rescheduled at least 24 hours before the scheduled appointment time.

Appointments canceled with less than 24 hours' notice, missed appointments, or failure to arrive for a scheduled appointment may be subject to a cancellation fee in accordance with Glow Med Spa's current cancellation policy.

Repeated late cancellations or no-shows may result in restrictions on future scheduling.

4. Deposits

Certain services may require a non-refundable deposit to reserve an appointment. Deposits will be applied toward the scheduled service unless otherwise stated.

If an appointment is canceled outside the required notice period, the deposit may be forfeited.

5. Payments

Payment is due at the time services are rendered.

Glow Med Spa accepts approved forms of payment, including major credit cards, debit cards, and other payment methods offered by the practice.

Returned payments or chargebacks may result in additional fees and suspension of future services until the balance is resolved.

6. Memberships

Membership terms, pricing, benefits, renewal dates, cancellation policies, and promotional offers are governed by each individual membership agreement.

Membership benefits are non-transferable unless specifically stated otherwise.

Unused membership funds or benefits are subject to the terms outlined in the applicable membership agreement.

7. Packages and Promotions

Treatment packages and promotional pricing are non-transferable unless otherwise specified.

Promotional offers cannot be combined with other discounts unless expressly stated.

Glow Med Spa reserves the right to modify or discontinue promotions at any time.

8. Gift Cards

Gift cards are redeemable for eligible products and services at Glow Med Spa.

Gift cards are non-refundable and may not be redeemed for cash except where required by law.

Glow Med Spa is not responsible for lost or stolen gift cards.

9. Retail Products

Retail products may be eligible for exchange or return only if unopened and in original condition within the timeframe established by Glow Med Spa.

Opened or used skincare products are generally not eligible for return due to health and safety standards unless defective.

10. Treatment Results

Clients acknowledge that aesthetic treatments vary by individual. Multiple treatments may be necessary to achieve desired outcomes.

Glow Med Spa does not guarantee specific cosmetic or medical results.

11. Client Responsibilities

Clients agree to:

  • Provide complete and accurate medical information.

  • Inform providers of any changes in medications or health conditions.

  • Follow all pre-treatment and post-treatment instructions.

  • Report any unexpected side effects or complications promptly.

  • Ask questions before consenting to treatment.

Failure to follow treatment instructions may affect results.

12. Photography

With client consent, clinical photographs may be taken for documentation, treatment planning, and medical records.

Separate written authorization is required before any identifiable photographs are used for marketing or educational purposes.

13. Mobile Terms of Service

By providing your phone number and email address, you consent to receive appointment reminders, scheduling updates, treatment follow-ups, billing notifications, membership information, and promotional communications from Glow Med Spa.

  1. Messages related to your account, orders, services, appointment reminders, marketing and promotional messages may be received. Frequency may vary.

  2. You can cancel the SMS service at any time by texting "STOP." After you send "STOP," we will confirm your unsubscribe via SMS. After this, you will no longer receive messages from us. If you would like to opt back in, you may sign up again as you did previously.

  3. If you are experiencing issues with the messaging program, reply with the keyword "HELP" for assistance, or contact us directly at misty@glowmedspa-camas.com OR 360-594-0532.

  4. Carriers are not liable for delayed or undelivered messages.

  5. Message and data rates may apply for any messages sent to you from us and from you to us. Message frequency may vary. For questions about your text or data plan, please contact your wireless provider.

14. Privacy

Glow Med Spa respects your privacy and protects your personal health information in accordance with applicable federal and state privacy laws, including HIPAA where applicable.

15. Minors

Clients under 18 years of age may receive services only with the consent of a parent or legal guardian and when permitted by applicable law and the provider's clinical judgment.

16. Limitation of Liability

To the fullest extent permitted by law, Glow Med Spa shall not be liable for indirect, incidental, special, or consequential damages arising from the use of its services, products, website, or facilities.

Nothing in this agreement limits liability where prohibited by applicable law.

17. Right to Refuse Service

Glow Med Spa reserves the right to refuse or discontinue services when deemed medically inappropriate, unsafe, abusive toward staff, fraudulent, or otherwise inconsistent with maintaining a safe and professional environment.

18. Changes to These Terms

Glow Med Spa may update these Terms of Service from time to time. Updated versions become effective upon posting or otherwise providing notice to clients.

19. Governing Law

These Terms of Service shall be governed by the laws of the States of Washington and Utah.

20. Client Acknowledgment

By scheduling services, signing treatment consents, enrolling in a membership, purchasing products or services, or using Glow Med Spa's website or patient portal, I acknowledge that I have read, understood, and agree to these Terms of Service.

SMS Terms & Conditions

1- SMS Consent Communication:
The information (Phone Numbers) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.


2- Types of SMS Communications:
If you have consented to receive text messages from Glow Med Spa, you may receive messages related to the following:

  • Appointment reminders

  • Follow-up messages

  • Billing inquiries

  • Promotions or offers (if applicable)

Example: "Hello, this is a friendly reminder of your upcoming appointment with Misty Shields at Glow Med Spa on 01/07/25 at 9:00. Reply STOP to opt out of SMS messaging at any time."


3- Message Frequency:
Message frequency may vary depending on the type of communication. For example, you may receive up to 2 SMS messages per week related to your appointments/billing, etc.
Example:
"Message frequency may vary. You may receive up to 2 SMS messages per week regarding your appointments or account status."

4- Potential Fees for SMS Messaging:
Please note that standard message and data rates may apply, depending on your carrier’s pricing plan. These fees may vary if the message is sent domestically or internationally.

5- Opt-In Method:
You may opt-in to receive SMS messages from Glow Med Spa in the following ways:

  • Verbally, during a conversation

  • By submitting an online form

  • By filling out a paper form

6- Opt-Out Method:
You can opt out of receiving SMS messages at any time. To do so, simply reply "STOP" to any SMS message you receive. Alternatively, you can contact us directly to request removal from our messaging list.

7- Help:

Additional Options:
